One-Page Profiles

One-Page Profiles are a practical tool used in person-centred practice to help ensure people are understood and supported as individuals. They provide a clear snapshot of who a person is, what matters most to them, and how best to support them.

Widely used across social care, education, and community services, One-Page Profiles help teams move beyond paperwork to develop a deeper understanding of the people they support. This workshop explores how One-Page Profiles can strengthen person-centred approaches, improve communication between teams and services, and ensure that support is shaped around what truly matters to each person.
